Miley is making his way back from Tommy John surgery and hopes to resume pitching in the majors before the end of May. The 38-year-old made just two appearances for the Brewers last season, but he was very good in 2023, going 9-4 with a 3.14 ERA in 23 starts. He had maybe the best season of his career with the Reds in 2021, going 12-7 with a 3.37 ERA in 163 innings.

Miley only had surgery in May, so that’s an aggressive timetable. Still, at age 38, he doesn’t feel he has much time to waste. Miley has struggled to stay healthy, but he’s usually been effective during the second half of his career. Since 2018, he’s 42-25 with a 3.46 ERA in 114 starts and three relief appearances.

Miley will receive a $1.5 million buyout and is now a free agent. He’s recovering from Tommy John surgery that will keep him sidelined for at least the first few months of the 2025 season. Miley turns 38 in November so it’s fair to wonder if he’ll even continue playing. He was 9-4 with a 3.14 ERA, 1.13 WHIP and 79 strikeouts in 23 starts for the Brewers in 2023, but went down with an elbow injury after only two starts in 2024. Miley has won 108 games with a 4.07 ERA in his 14-year career. He made the All-Star team and was second in the NL Rookie of the Year voting in 2012.

Miley landed on the injured list on Monday due to inflammation in his left elbow. We now know that the situation is much more dire. He’ll miss the remainder of the 2024 season — and likely the first few months of the 2025 campaign — while recovering. That is if he even decides to continue his playing career. It’s a brutal blow for the 37-year-old southpaw.

Another pitcher down for the Brewers. Miley allowed four runs (three earned) in three innings against the Padres last time out while also getting hit in the knee by a 108 mph comebacker. However, it’s the elbow that’s ailing him. We should know more specifics in the coming days. Tobias Myers is on his way up from Triple-A.

Miley limited Cincinnati’s solid lineup to just one run on one hit and a pair of walks, but also failed to record a strikeout in his season debut. Given his lack of strikeout upside, he’s unlikely to provide much of an impact for fantasy managers outside of extremely deep mixed leagues and NL-only formats. He’ll face the Padres on Monday in his next outing.

The 37-year-old will make his season debut against the Reds on Wednesday. Miley started the season on the injured list with a left shoulder impingement but the team could be cautious with him in his first outing since he only threw around 55 pitches in his final rehab start for Triple-A Nashville. Fantasy managers could stash him and see how he performs in his first outing before putting him in lineups, but his health is a big question mark at this point in his career.
